Is it free to create an online health passport?

Yes, it’s free to register and easy to use.

Is my data secure?

Keeping your health information safe and secure is very important to us.

If you are a patient of Bradford District Care NHS Foundation Trust, we are responsible for your data. We only share information with the people who need it to provide your treatment and care.  If you choose to register for an online health passport, only authorised people involved in your treatment and care will be able to see your data. You can choose to opt-out at any time.

Where will my online health passport data be kept?

We use Patients Know Best (PKB) software. This meets NHS requirements for health record systems and is compliant with the General Data Protection Regulation (GDPR).

Only the minimum level of data needed will be uploaded. All personal and health information stored is encrypted.

Will I need an email address to register?

Yes. you can only use one unique email address per registration.

If you share an email address with other family members or your carer, and they already use that email for their own Patients Know Best (PKB) account, you will not be able to use that email address to register.

If you register for an online Health Passport using the NHS app, it will automatically use the email address linked to that account.

Do I need internet access to use my online health passport?

Yes. This is an online service and will require you or a carer to have internet access to use your online health passport.

I’ve registered – why can’t I see my health information?

Some patients may not have any information in their records when they first register, however this will change over time as you visit the Children in Care team. You can still add your own information, such as tracking your symptoms or recording any medication you take.

What happens if I don’t want to register?

It is your choice if you want to access your health information online. You can ignore invitations to register if you choose not to use this method.

Can I give a carer or family member access to my online health passport?

If you want the family member or carer to have access to your online health passport they can be added in the ‘sharing’ section. This will allow them to set up their own account. You can control the level of access they have to your data. Find out more about sharing your record with a carer or family member.
